This Application form serves as your “Wish List” and helps me to select the ideal Labradoodle puppy for your family situation and lifestyle. ADOPTION AGREEMENT AND HEALTH WARRANTY

Lora Dowd, Hereinafter named Breeder, offers for adoption the below described Labradoodle, under the conditions of the following agreement, for the sum of $2500.00 + shipping charges which may Vary due to destination and size of puppy and crate. You will be notified of our shipping amount.

Upon receiving said dog, buyer agrees to terms listed below.

Make a copy of this agreement.

1.                  Buyer agrees to take the proper safeguards for the care, safety and well-being of the dog sold herein, including but not limited to a fenced yard or proper kennel, adequate shelter, feeding and exercise, socialization, and of special importance, Obedience Training.

2.                  Buyer further agrees that if for any reason in the future you do not wish or are unable to keep this dog, Breeder is to be notified and dog may be returned, at buyer’s expense and no charge to the breeder. (No money will be returned to buyer at any time if said dog is returned).

3.                  Breeder is to be notified of Buyers change of address and phone number for her records.

4.                  The said dog is warranted to pass OFA hip and elbow certification, within one (1) month of this dog reaching two (2) years of age.  In the event said dog is suspect to a Genetic Disease, presentation of medically supported evidence verified by two qualified veterinarians that the dog is physically deficient and not functional for its intended purpose, must be supplied to the Breeder.  If said dog is suspect to hereditary dysplasia, radiographs must be submitted to the Orthopedic Foundation for Animals (OFA) and evaluated.  Said dog is also warranted against hereditary eye disease.  Should said dog fail to pass either OFA hip and elbow certification the Breeder offers the following warranty:                                                                                                   

The Breeder agrees to replace said dog with one of equal value within a reasonable length of time.  All veterinary and shipping costs are at the Buyer’s expense.  Breeder will replace said dog with any color or gender or age at breeder’s choice and discretion. If replacement is denied it is breeders right to void this warranty and not replace dog. Breeder has the right to have the deficient dog returned to breeder at buyer’s expense and at the breeder’s discretion. Buyer agrees that at no time will the dog ever be rendered to an animal shelter.

5        The above warranty applies only to the below named purchaser who is also the original recipient of the dog.

6.                  Feeding a base diet of generic, common or grocery store dog food voids this warranty. Allowing the said dog to become overweight and obese voids this warranty. Full warranted nutrition is available on my web site for reading and researching. Breeder will send information with the puppy on recommended food as well.

7.                  Breeder will not replace a puppy or refund money for a dog that develops behavioral problems. The puppy should start full obedience training with in two (2) months of the buyer receiving the puppy and must complete his training and graduate from FULL obedience classes, not just puppy classes. The dog must be properly socialized with adults, male and female, children and dogs other than family members and pets for the lifetime of the dog.

8.      Breeder has the right to have the dog returned to Breeder at Buyer’s expense if the dog is no longer wanted for any reason.  No money will be refunded to the buyer at any time. Breeder also has the first right of refusal to receive said dog back, and will advise buyer to rehome said dog and at buyers expense if the breeder refuses dog to be returned. 

9.   Buyer agrees to never let the puppy out of the shipping crate until they are home and behind a safe closed fence. Any dog who gets lost or injured at any time after buyer takes possession of dog, will not be replaced and money will not be refunded.
